# Hypertension Clinical Trials — Patient Guide

If your high blood pressure isn't well controlled despite medication, a clinical trial may be an option. This guide explains what trials are studying and how to know if one fits you.

## What hypertension clinical trials test

- New medications for uncontrolled or resistant hypertension
- Novel approaches like RNA interference (e.g., zilebesiran from Alnylam)
- Combination therapies and dose-finding studies
- Treatments for specific hypertension subtypes (resistant, secondary, malignant)

- **ZENITH (zilebesiran)** — Alnylam ALN-AGT01-008, Phase 3 hypertension. https://www.amavitaresearch.com/trials/zenith-zilebesiran

ZENITH is studying a long-acting RNAi therapy that may control blood pressure with a single subcutaneous dose every 6 months — a different approach from daily oral medications.

## Who typically qualifies for a hypertension trial

- Diagnosis of hypertension (uncontrolled or resistant defined by the protocol)
- SBP ≥140 mmHg despite ≥2 medications (uncontrolled) or ≥3 medications including a diuretic (resistant)
- Adult (specific age range varies by trial)
- No recent cardiovascular event within a defined window
- Willing to undergo 24-hour ambulatory blood pressure monitoring (ABPM)

## What patients can expect

- 24-hour ABPM during screening and at protocol-defined intervals

## How 24-hour ABPM works

A small monitor is fitted at the site. It takes blood pressure readings every 15–30 minutes for 24 hours while you go about normal activities. You return the device the next day. ABPM is more accurate than office measurements and is required for many hypertension trials.

3. Screening visit including ABPM
4. PI reviews and confirms eligibility
5. Informed consent — take it home, read it, ask questions, decide
