# For Referring Physicians

A guide for primary care physicians, general cardiologists, and specialists in South Florida who want to refer eligible patients to an active cardiovascular clinical trial at Amavita Research.

## Why refer a patient to Amavita Research

- Single site, single PI per protocol — clear accountability for the referring physician
- Active enrollment across heart failure, hypertension, peripheral artery disease, structural heart, AFib, acute coronary syndrome
- Patients retain their primary cardiologist; Amavita Research coordinates trial-specific care
- Free trial-related care (study drug, study procedures, study visits)
- Bilingual (English/Spanish/Haitian Creole/Portuguese) coordinators

## What happens to a referred patient

1. Patient or referring physician contacts Amavita Research (786-703-5941, research@amavita.health)
2. Coordinator screens for trial fit during a brief intake call
3. If a potentially fitting trial exists, patient is invited for a screening visit
4. PI reviews and confirms eligibility
5. Patient enrolls if all criteria are met; care plan is shared with the referring physician
6. Patient continues seeing their primary cardiologist for non-trial care
7. End-of-study summary is sent to the referring physician

## Patient populations Amavita Research is currently enrolling

- Heart failure (HFrEF, HFpEF), iron-deficient HF
- Hypertension (uncontrolled, resistant)
- Peripheral artery disease (claudication, critical limb)
- Acute coronary syndrome (post-event optimization)
- Structural heart disease consultation
- Atrial fibrillation device protocols (via EP sub-investigator)

## Co-management approach

Amavita Research does not replace a patient's primary cardiologist or PCP. The referring physician continues to provide routine care and receives:

- Visit summaries on a quarterly basis
- Notification of any SAEs
- End-of-study summary with the patient's outcomes
- Continued access to discuss the patient's care

## Liability and credentialing

- Referring physicians are not investigators on the trial; no FDA Form 1572 obligation
- No transfer of medical malpractice liability — Amavita Research carries its own coverage
- No financial relationship — patient referrals are not compensated
